Wouldn't it be better to have this place named "Ad Confluentes" like most names with the 'Ad' prefix?

Only if the evidence backs it up. I checked Manandian 1965 and it actually does attest to Adconfluentes: http://books.google.com/books?id=PNRAAAAAIAAJ&q=adconfluentes.

I'm guessing the only ancient source attesting to this name is the TPeut (http://www.cambridge.org/us/talbert/talbertdatabase/TPPlace2486.html), and both Manandian and BAtlas are being conservative in following its approach. I can't get at a copy of Manadian easily at present, but I'll follow up with Richard Talbert and see if he has any guidance.
